The one issue I can see with this, is the nested nature of categories. If this were to work, the output would be linear only, ie. parent/child categories would all be treated equally. That might actually be a half-baked solution, which I don't like. Will ponder on it some more.
I see what you're saying. Right now, the hard-coded version I'm using as a reference uses the primary category for listing the alphabetical heading.
I'm using GWCode Categories to output the list and assign different tags to the primary and secondary categories but can't add an alphabetic header...and figure out the sorting biz.
